What details do you need to provide?

For us to review you case and issue you with a travel and cancellation letter, we’ll need the following information;

  • A photo of your ID (could be a passport, driving license or workplace ID card)
  • The sickness or medical condition that has affected your ability to travel
  • Confirmation of when your sickness or condition started - if you have been admitted to hospital upload a discharge letter you received.
  • A short video stating only your first name and confirm the service requested (e.g. "My name Nadia and I am requesting a travel and holiday cancellation letter".)**
  • (Optional) If you want us to contact your regular NHS GP about your consultation, have ready the details of your GP practice.
